Passport Application Requirements:
- One 2″ by 2″ passport photo
- U.S. birth certificate (original or certified copy) or naturalization papers
- Need a Birth Certificate?
- Visit Healthy Arkansas for a certified copy of your Arkansas birth certificate.
- Visit Vitalchek for an all states listing.
- View Arkansas Vital Records
- Completed passport application
- Visit NPIC for online forms, printables and more.
- Fill out form in black ink and don’t sign until you are asked to do so in office.
- Driver’s license or a state-issued I.D. card
- Visit AR DFA for driving & identification documents
- For children ages birth to 15:
- signatures of both parents
- child(ren) must be present at time of application
- both parents or childs legal guardian must appear in person with valid ID
- Special CircumstancesIf there are any special circumstances (i.e. birth abroad, only one parent available, lost or expired passport, etc.) there may be additional forms or documentation necessary. Please ask a clerk or contact our offices at 479-271-1017
- For children ages 16 & 17:
- signatures of one parents (child’s signature is required)
- child(ren) must be present at time of application
- one parent or legal guardian must appear in person with valid ID
